PHSS Support Services is a non-profit organization located on Colborne Street in Central London. The organization serves individuals with developmental, medical, and complex physical needs, providing them with the tools to live independently and with dignity in their own homes and communities. Their mission focuses on fostering inclusion, quality care, and a sense of belonging for those who require specialized support. The organization offers a wide range of family-centred and personalized supports designed to adapt to changing needs over a person's lifetime. Specific services include 24/7 supportive housing, residential supports, long-term ventilation, and intensive supports such as day respite and overnight respite. They also provide community supports like recreation activities, passport management, and family resources. Additionally, PHSS offers professional services including training and education, consultation, and supports and services design and evaluation. A core component of their approach is the development of Person-Centred Plans, which serve as roadmaps for goals involving health, transportation, home and personal care, and social activities. Founded in 1988 by a small group of families and advocates, PHSS began as a grassroots initiative to provide opportunities for people with disabilities to live near their loved ones rather than in institutions. The organization rebranded in 2016 to reflect its growth in supporting increasingly complex medical needs while maintaining its original values. They operate under a no-discharge policy, making a lifelong commitment to those they support. PHSS currently maintains over 80 locations across the province and supports more than 450 individuals.
